entropy_source_rec_thresh_lut
Map MIN_ENTROPY_H (Q4.4) to SP 800-90B recommended APT and RCT cutoffs.
Pure combinational LUT from gen_recommended_thresholds.py; regenerate with that script. RCT_LIMIT is C = 1 + ceil(20/H) from SP 800-90B 4.4.1; APT_LIMIT is 1 + CRITBINOM(W=1024, 2^-H, 1-2^-20) from 4.4.2. SEP DV re-derives both formulas independently from the standard and does not read this table.
