sep_local_axi_xbar_wrapper

Adapt sep_pkg AXI structs to the local crossbar request and response types.

Initiator (slave-into-xbar) ports use 3-bit ID masters. Target (master-from-xbar) ports use 6-bit ID slaves. The crossbar test mode is tied to 0.
